School Forms

Uniform Form - If you would like to place an order for school uniform please make your request on a School Uniform Form. To find out more about the school uniform click here.

Admission Form - If you would like to apply for a place for your child at St Peter and St Paul's Church of England Primary School please complete and submit the the admission form.

Holiday Form - If you wish to make a request for a leave of absence for your child from school, requests need to be made in advance of the proposed dates. Please Note: SATS Test Week during May for Y2 & Y6 - no holidays will be authorised . School holiday dates can be found here.

After School Club Permission Slip - To enable your child to attend any of the after school clubs you need to have returned to school a signed After-School Club Permission Slip. To view the current term clubs please click here. Permission Slips can be obtained from the School Secretary or can be downloaded here . Once completed please hand into the school office as soon as possible.
